Begin a Mediation

We're glad to help.  Please call us at 707-572-4092 if we can answer questions.
Our Case Managers can assign mediators to your case when each of the people who would like to mediate have submitted an intake package of 3 items

(1) an intake questionnaire,
(2) a fee schedule, and
(3) fees.

All can be completed online or by postal mail.  If you would like our Case Manager to invite the other person or persons to mediate, please submit your documents and fees and provide contact information for the other person(s).  When we've received your forms, we'll get in touch to check in with you and answer any further questions.

How much does it cost?  Your fees will be based on a sliding scale, and we think you'll be happy to find they are not expensive.  Click here to view our sliding fee scale.  If our fees pose a hardship, please let us know; you will not be turned away because of inability to pay.

1.  To complete an intake questionnaire:  Please use one of the bulleted options just below.

  • HMS Intake Questionnaire - Online Intake Questionnaire
Probably the quickest way is this first option - Online Intake Questionnaire.  If you prefer an additional level of online privacy, please call our office at 707-572-4092 to get a file number to use on the form in place of your name.
  • HMS Intake Questionnaire - PDF
You can download the PDF version, complete the form, and return it either by post or an attachment to an email.

2. To complete a fee schedule:  Please use one of the bulleted options just below. When you've completed your fee schedule, jot down the fees for your row on the sliding scale so you'll have this information when you pay your fees in #3 below.

  • HMS Fee Schedule - Online Fee Schedule
Probably the quickest way is this first option - Online Fee Schedule.  If you prefer an additional level of online privacy, please call our office at 707-572-4092 to get a file number to use on the form in place of your name.

You can download the PDF version, complete the form, and return it either by post or an attachment to an email.

3.  To pay fees:  Please complete the form below based on your fee schedule in #2 above.  Fees for unused case development or mediation sessions will be returned.

Note:  The fee schedule linked in #2 above is for interpersonal mediation.  If you represent a business or organization and your gross annual revenue is $250,000 or greater, please contact us for a quote - our sliding scale for businesses and organizations goes up incrementally where this interpersonal fee schedule leaves off.

The Mediation Process:

  • A Case Manager explains mediation process and answers questions
  • Each person/party completes an intake package (see above)
  • A Case Manager assigns a panel of 2 mediators
  • 1 of the mediators contacts the parties to schedule individual case development interviews with each party.  Both mediators assigned to your case will be present and facilitate the case development interviews.
  • The panel of 2 mediators and the parties meet for a mediation, working toward a win-win written agreement for going forward.  The agreement is decided by the parties themselves.  HMS mediators don't judge, preach or recommend any particular solution, and they don't represent one side or the other.  They will guide you through a sequence of steps, hold the ground rules, ensure interpersonal respect, and work to "level the playing field."  Each party will have opportunities to speak without being interrupted.  A session can last up to 3 hours.  Coming to agreement can take one session or many sessions; the choice is up to you, the participants.  You own the process!

Excerpts from Humboldt Mediation's Consent to Mediate:

We agree to the following:

  • We will use words and actions that are respectful and courteous.
  • We will speak honestly, in good faith, and will share all information that will help us work toward mutual understanding.
  • We will listen respectfully, giving each person a chance for full, uninterrupted expression of his or her options, and we will seek to learn from all perspectives.
  • We will work together to explain and understand our interests and needs, to explore options, and to create our own solutions.
  • We will keep confidential all information gained in the mediation; we will not use anything we have learned against any other person.  We agree, however, to the following exceptions:
    *  If any of us has reason to believe that there is an immediate threat to human safety, we may disclose the threat if this appears necessary to prevent someone from being injured.
    *  Statements or evidence of child or adult abuse.
  • If both parties agree, we may submit our written agreement to the Court.
  • We will not involve the mediators, staff, or records of Humboldt Mediation Services in any court proceedings or administrative hearings.
  • We will honor any agreement that we may sign.
